Every year, over 2000 autistic people and people living with a learning disability remain in inpatient facilities, hoping to return to their communities. Without timely and personalised support, people can experience fragmented care, often prolonging their challenges and limiting their progress.

Our tailored Community Transition Services break down barriers to discharge,
supporting independence and enhancing quality of life.

We work in partnership with Integrated Care Boards, local authorities, and other care providers for effective discharge support.
