From Hospital to Home, with Care You Can Trust
Every year, over 2000 autistic people and people living with a learning disability remain in inpatient facilities, hoping to return to their communities. Without timely and personalised support, people can experience fragmented care, often prolonging their challenges and limiting their progress.
Our tailored Community Transition Services break down barriers to discharge,
supporting independence and enhancing quality of life.
We work in partnership with Integrated Care Boards, local authorities, and other care providers for effective discharge support.

Ensure a smooth transition to the new home and provide all the necessary support.
Work in partnership with the new provider to ensure the right match for each person.
Gradually withdraw our support team.
Provide assitance and support to the person and the new provider until we ensure that the transition is safe and well established.
Discharge

We are here to support care providers in delivering the right care, identifying unforeseen challenges, and providing assistance for as long as necessary.
