Luke’s Transition: The power of compassionate and consistent care

Luke is a remarkable autistic boy who faced numerous challenges in communicating his needs and desires. Previously, he had experienced multiple unsuccessful placements, often due to property damage. Despite these setbacks, Luke now lives independently and is achieving goals once deemed unattainable.

However, a recommendation led to Nurseline Healthcare’s Community Transition Services getting in.

When we first met Luke, he was under the care of another provider who struggled with his behavior, largely triggered by his living environment and health issues. We provided transition support, suggested improvements to his support plan, and, despite the provider’s notice, we were committed to finding a suitable solution for Luke.

Our team worked tirelessly with Luke’s family and a progressive local authority and Clinical Commissioning Group to secure a new home for him. With our support, Luke transitioned safely to a new care provider and continues to thrive in his own home.
